  • It cannot be used in conjunction with another dental plan, dental insurance, or financing program.
  • This plan is only honored at Castro Valley Dental Home. It is not used at any other dental or specialist office.
  • Patient must pay in full at the time of service to receive the courtesy discount.
  • No refunds of premiums will be issued at any time if participant decides not to utilize the dental plan.
  • All fees are non-refundable.
